Community health workers conduct needs assessments, engage in collaboration, and evaluate programs to empower communities and address health disparities.

Community Impact Assessment

We take into account honest drivers of health, our Community Health Workers (CHWs) are culturally aligned, and uplifting community assets while advocating for better health outcomes.

Health Services

Public health services can provide a range of resources to the community. Community Health Workers assist in navigating public health services to increase their impact.
